Kids Church & Youth Group Registration – Agreement

• Safety & Supervision: Our team is committed to providing a safe and welcoming environment. Children and youth must follow the instructions of leaders at all times.  
• Respect & Behavior: We expect all participants to treat others with kindness and respect. Disruptive or unsafe behavior may result in dismissal from the program.  
• Health & Medical: Parents/guardians must inform us of any medical conditions, allergies, or special needs. In case of emergency, we will follow the provided emergency contact details however we reserve the right to contact an ambulance or doctor at our discretion. 
• Photos & Media: We may take photos/videos during activities for church purposes. Please notify us if you do not consent to your child being included.  
• Drop-off & Pick-up: Children must be signed in and out by a parent/guardian unless prior arrangements have been made.
• Liability: While we take every precaution for safety, the Church is not responsible for personal items lost or damaged during activities.
